$75 | Postpartum exam | Taking care of yourself after your baby is born includes seeing your doctor for a postpartum visit within 7 and 84 days following delivery. The reward is loaded to your CARE card after the claim is submitted by your provider and can be earned once following your pregnancy. |
| $75 | Prenatal visits | It is important to start prenatal care, or care during pregnancy, as soon as you know you are pregnant. It’s also important to go to all of your prenatal visits. These visits help you and your baby stay healthy. Learn more about our special program called Bright Start for pregnant and new moms. We will add the reward to your CARE card after your provider sends us the visit information for any one prenatal visit. This may not happen until after your baby is born. |
| $75 | Breast cancer screening | Members ages 40 – 74 can earn $75 for getting one mammogram each year. A mammogram is a breast cancer screening. Many people should start getting mammograms at age 40. A mammogram can find changes in your breasts before you can see or feel them. If you haven’t scheduled your mammogram, please call your provider today. If you need help finding a provider, please call our Member Services department at 1-855-375-8811 (TTY 1-866-209-6421). We will add the reward to our CARE Card after your provider sends us the visit information. You can earn this reward one time each fiscal year. |
| $75 | Diabetic retinal eye exam | Did you know that diabetes can affect your eyesight? Ask your provider about eye exams for diabetes at your next visit. Members with diabetes can earn $75 for getting a diabetic retinal eye exam from a vision provider. We will add the reward to your CARE Card after your provider sends us the exam information. You can earn this reward one time each fiscal year. |
| $75 | Child well visit 0 – 15 months | Children from birth through 15 months who are AmeriHealth Caritas North Carolina members can earn up to $75 total for completing six or more well visits before they turn 15 months old. At these visits, your child gets a full checkup, no-cost vaccines, and vision and hearing tests. Learn more about well-child visits. We add rewards to your child’s CARE card after your provider sends us the visit information for each qualifying visit: the first, third, fifth, and sixth visits. |
